CHECK OR MONEY ORDER VIA MAIL
- Click on the ORDER FORM button. A printable Order Form is presented in a PDF format which requires Adobe Acrobat Reader.
If you do not have Adobe Acrobat Reader, you may download the progam free of charge from the Adobe web site by clicking on
the Adobe icon.
- Fill out the Order Form with your name and complete mailing address. Please print ledgibly.
- Include your e-mail address in the event I would need to contact you about your order.
- Please add shipping charges using the table on the bottom left of the Order Form.
- Make check or money order payable in US dollars only to Betty Waldbieser and mail to
1584 Fourth Street, Bethlehem, PA 18020 USA
- Booklets will be shipped within 48 hours of receipt of payment.
- Pay Pal is an online secure pay-by-credit card service. Pay Pal accepts all major credit cards including
Visa, Mastercard, Discover and American Express. All transactions are completely confidential.
- To purchase an item click on the "Add to Cart" button. To view the items in your cart click on the "View Cart" button.
When you are finished shopping, click the "Checkout" button. The only thing left to do is enter your payment info.
